Period ink notation reads, "#453 Grand Slam Warren Spahn 5/20/65" (end of date is very faded). The Braves played the Mets at Milwaukee County Stadium and Mathews hit his 453rd career homerun off Hall of Famer Spahn in the 5th inning, a Grand Slam.
