NM, Questionnaire FR) Sheet of 5.5"x8..5" paper sent to Vance by a reporter has a series of pre-printed question which the user was requested to fill in. It has been signed at the top "Arthur Dazzy Vance" in pencil rating 8 out of 10. He has handwritten his answers throughout and these include his address, place of birth, nickname, position, first pro team, other pro teams, and a few additional points. Displays at better level than technical grade might indicate. There are chips and small tears in the edges. There are also a few tears and creases/wrinkles. Includes full LOA from JSA: Sig.
