Description: December 22, 1971 Roberto Clemente full name signed sworn statement. Original sworn declaration as filed by Roberto Clemente in order to secure a replacement driver's license. Exceedingly rare "Roberto Clemente Walker" full name signature is done in ink rating 8/9 out of 10 and was witnessed by a Notary Public. Light toning to thin carbon copy stock with fold lines from storage and some general wear including a handful of staple holes near left edge. This is more accentuated along the lower fold. There is a small tear to upper right corner and some other chipping. Includes full LOA from JSA and letter of provenance from the Clemente Family: Sig.
