Description: Fine 1978 Ozzie Smith handwritten questionnaire. Legal sized questionnaire has July 18, 1977 date stamped at upper corner. Smith has signed his full name "Mr. Osborne Earl Smith" at top and handwritten answers to a number of personal and baseball related questions with some choice content. Most notably, he relates his most exciting moment was being named MVP in the World Amateur Baseball Tournament in 1976. Smith had just then been signed to the San Diego Padres and would debut the following the year embarking on his Hall of Fame career. All done in ink rating 8/9 out of 10.
