Description: Mel Ott photograph and questionnaire with handwritten notations/answers. Includes scrapbook page with 5"x8" sepia toned photo affixed on which Ott has written, "This is the way I get in shape." Also on the page are some publication page clippings and an additional 8"x10" vintage photo; multi-page questionnaire as sent to Ott in 1957 which he has filled in on the first (2) pages in block letter print. None of the pieces is autographed. Handwriting rates 7 to 8/9 out of 10. Light scattered wear.
