Description: 1971 Goose Gossage handwritten questionnaire. Legal sized questionnaire has Dec. 13, 1971 date stamped at upper corner. Gossage has handwritten his full name "Richard Michael Gossage" at top and handwritten answers to a number of personal and baseball related questions with some choice content. Most notably, he relates his greatest thrill includes pitching an inning in a White Sox vs. Cubs exhibition game. All done in ink rating 8/9 out of 10. Faint fold lines and a touch of light wear.
