Description: 1946 Eddie Waitkus handwritten questionnaire. Legal sized "American Baseball Bureau" questionnaire has Feb. 12, 1946 date stamped at upper corner. Waitkus has signed his full name "Edward S. Waitkus" at top and handwritten answers to a number of personal and baseball related questions with some choice content.
